We are a non-profit ministry organization that owns and manages medical practices and specializes in transitional care.
By working directly with hospitals and local physicians, we ensure that transitions from hospital to home are as smooth as possible with no gaps in patient care thanks to our holistic approach to recovery.
Our nurse practitioners attend to your medical and physical needs, but we recognize that recovery is not one-sided—it includes the mental and spiritual too. That’s where our ministry team step in to carry out our mission.

OUR VISION
To provide an entire team of dedicated professionals that assist our facility partners in managing their patients for 30 days post discharge, to support a healthy recovery, and to reduce readmissions.
