At Segue Health, we are committed to serving you and your loved ones to help ensure a smooth recovery. While we will do our best to ease the transition from hospital to home, there are many things that you can be doing at the same time to speed up your recovery.
We spoke with a few of our nurse practitioners, and they recommended these three tips as some of the most effective in improving recovery time.
- Compare your discharge Rx list with the medicines you have at home to make sure you are taking the right medicines and the right doses. Sometimes the doctor will take you off some of the medicines when you’re in the hospital or change the dosage.
- Follow up with your PCP because this is crucial.
- Before you leave the hospital, read the discharge papers and ask questions if you don’t understand something.
All of the above tips are great, but one of the most important out of these is close follow-up with your Segue nurse practitioner and your PCP.
